The practitioners at Indigo Integrative Health Clinic have been extensively trained to be able to order and interpret the results of the functional testing we order. These tests require special education and advanced knowledge that many conventional practitioners do not have.
Our practitioners will order lab testing only after an appointment with you that includes a discussion with you about your concerns, symptoms and medical history. You are required to return for a Follow-Up visit to discuss your results for all labs you perform with our practitioners at Indigo Integrative Health Clinic, PLLC. Results will not be released to you prior to your consultation with your practitioner. Results will also not be released to you if you choose to discontinue care at our office. We are only able to release the information to a new practitioner of your choice, who will then take full responsibility over your care and it will be at their discretion when you receive your results.
Indigo Integrative Health Clinic, PLLC is not a lab testing center. We order labs tests for established patients only.
If more than one specialty lab is performed, you will need more than one follow-up appointment to review the results. Follow-up appointments are 60-minutes or 90-minutes long in order to allow ample time to review results, determine which treatment options are appropriate for you and to answer all of your questions. We do not provide 30-minute appointments, unless there is an acute need.
It is at your discretion to share the results with other providers. It is your responsibility to keep a copy of these results that we provide you. We are unable to fax copies of these reports to other practitioners unless it has been discussed prior to having the test done. You can obtain hard copies of these results for an administrative charge.
All blood draws performed at Indigo Integrative Health Clinic, PLLC will have a process and handling fee of $100. More specialized lab tests may have a different fee, which will be discussed with you prior to lab draw.
Please be advised your insurance company may not approve reimbursement for the functional medicine labs. Do not rely solely on insurance reimbursement when considering labs. If necessary, choose the most pertinent labs first.

INFORMED CONSENT FOR FUNCTIONAL MEDICINE LABORATORY TESTING
The purpose of functional medicine laboratory testing in our office is to evaluate nutritional, biochemical, or physiological imbalance and to determine any need for medical referral. These lab tests in our office are not intended to diagnose disease. This office utilizes conventional lab tests as well as functional medicine assessment.
Functional medicine assessment is designed to assist our practitioners and other healthcare providers in finding the underlying causes of your condition. Functional medicine has evolved through the efforts of scientists and clinicians from the fields of clinical nutrition, molecular biology, biochemistry, physiology, genetics, conventional medicine, and a wide array of scientific disciplines. Functional medicine evaluates the body as a whole, with special attention to the relationship of one body system to another and the nutrient imbalances and toxic overload that may adversely affect these relationships.
Your conventional medical provider may or may not agree with the necessity for—or our interpretation of—these tests. If you have any questions or concerns, please discuss them with our practitioners.
